This form is for CMAC members who wish to reserve equipment or facilities for a new project or production.
Once submitted, you will hear back from CMAC’s Community Media Coordinator within 1 week.
Please note that it may be necessary to meet with you by phone or in-person to go over your proposal prior to approval. We look forward to helping you make your production a reality!
